Through case management, we offer timely, ongoing, comprehensive support and advice to individuals who are seriously ill and/or have been the victim of an accident. The goal is to facilitate a speedy recovery as well as long-term reintegration into everyday life and the working world, whether that means they return to their traditional occupation or take up a new, more appropriate activity.

Meaningful, Achievable Goals

Case management is geared toward the individual. It utilizes the resources and options available to the person concerned, while taking into consideration everything around them. Together with the partners involved (the individual, doctor, employer, relatives, cost bearer and service provider), the case manager sets meaningful and achievable goals.
